What is the Sliding Fee Discount - A program designed to reduce the cost of primary care for those who meet income requirements. These low fees are available to patients based on the guidelines provided annually by the federal government.
How Do I Qualify- You qualify for the program if you meet the eligibility guidelines. Sliding fee discounts are determined according to household size and yearly income, regardless of health insurance status.
We promise to understand. We recognize that not all fee-for-service balances are due to a patient’s unwillingness to pay, as there is often an inability. This practice services all patients regardless of ability to pay.

Certain medications necessitate periodic laboratory testing to confirm that they are reaching the appropriate concentrations in your blood. We provide a well-equipped laboratory for collecting blood and specimens for patients undergoing such testing.

It may not just be "being forgetful" or "absent-minded". We offer advanced memory testing that can catch early signs of Alzheimer’s and dementia—right here in our clinic. Telehealth
Because we’re in a rural area, many people have a hard time getting to the doctor. Some don’t drive and have to rely on a family member for a ride—but that person may be working or unavailable. To help with this, we offer telehealth (video visits) so you can still talk to your provider and take care of your health until you’re able to come in for an in-person visit.
Incontinence Care
We offer non-surgical, noninvasive treatment for urinary incontinence in women and men. Utilizing a safe and effective device that is FDA-cleared to strengthen a weakened pelvic floor. If you experience bladder leakage, loss of bladder control or frequent bathroom trips our treatment might be the solution.
