We welcome most private insurance plans. Feel free to contact our office or your insurance provider for confirmation.

If you don't have insurance, worry not! Our case manager is available to assist you in assessing your eligibility for programs like Medicaid, Marketplace insurance plans during active enrollment periods or others options. Additionally, we provide a sliding fee discount program based on household size and income. Get in touch with us for further details.

Discounts through the sliding fee scale are accessible for eligible individuals. Meeting specific criteria and submitting required documents are necessary. The computation of sliding fees depends on household size and income. If you do not meet the criteria for the sliding fee discount or lack the necessary documentation, you are accountable for the full payment. We accept cash, checks, and credit cards as forms of payment.

According to the law, healthcare providers are required to provide patients without insurance or those not using insurance with an estimate of the expenses for medical items and services.

Here are key points to note:

  • You have the right to obtain a Good Faith Estimate for the total expected cost of any non-emergency items or services, encompassing related expenses like medical tests, prescription drugs, equipment, and hospital fees.
  • Ensure that your healthcare provider furnishes you with a written Good Faith Estimate at least 1 business day before your scheduled medical service or item. You can also request a Good Faith Estimate from your healthcare provider or any other chosen provider before scheduling an item or service.
  • If you receive a bill that exceeds your Good Faith Estimate by at least $400, you have the right to dispute the bill.
  • It is advisable to retain a copy or take a picture of your Good Faith Estimate for future reference.

Getting Ready for Your Appointment

Newcomers should arrive at least 30 minutes prior to their scheduled appointment to complete the registration process, fill out necessary paperwork, confirm insurance details, and enroll in the patient portal. For those already in our system, arriving at least 15 minutes before the appointment is recommended to review and update existing patient information, verify insurance coverage, and complete any necessary paperwork. You can opt to receive appointment reminders through phone, text, or email, and it is crucial to confirm your appointment upon notification.

Ensure you have the following documents: a driver's license or another form of photo ID, your current health insurance card, and a list of your current medications. If you wish to explore eligibility for our sliding fee discount program, please bring along proof of income and details about your household size.
